Ketoconazole is a broad-spectrum antifungal medication, sold as both an oral tablet and a topical agent, which is used to treat fungal infections of the skin and nails. The drug is marketed under the brand name Nizoral by Johnson & Johnson. Nizoral works to weaken and kill fungus by blocking an enzyme that makes up a key component of the fungal cell membrane.
In July 2013, the FDA issued a Drug Safety Communication warning that ketoconazole tablets had been associated with severe liver injuries that may necessitate a liver transplant or even lead to death. The agency advised patients using the tablets to be aware of the symptoms of Nizoral liver injury, including:
- Loss of appetite, nausea, vomiting, or abdominal discomfort
- Fever, feeling unwell, or unusual tiredness
- Yellowing of the skin or the whites of the eyes (jaundice)
- Unusual darkening of the urine or lightening of the stools
- Pain or discomfort in the right upper abdomen, where the liver is located
The FDA’s ketoconazole warning also revealed that use of the tablets had been associated with adrenal gland problems and dangerous drug interactions. Symptoms of Nizoral adrenal gland problems may include:
- Fatigue
- Weakness
- Nausea, vomiting
- Diarrhea
- Loss of appetite
- Weight loss
The FDA has warned that the use of Nizoral is contraindicated with dofetilide (Tikosyn), quinidine, pimozide (Orap), and cisapride (Propulsid). Ketoconazole drug interactions can include dangerous heart rhythm problems, according to the agency.
The FDA’s 2013 Drug Safety Communication warned that ketoconazole tablets should never be used as a front-line treatment for ANY type of fungal infection. The agency also ordered the manufacturer of ketoconazole tablets, including Nizoral, to add a “Black Box” warning - the most serious type of safety notice - to their labels regarding the association with liver injuries, adrenal gland problems and dangerous drug interactions.
